    Hello, I have some forms that I would like to use in a custom page template. However I can’t seem to get the template page to work right. Either my code shows errors OR I dont get the template to show up.

    Here is the code I’m using right now.

    <?php
    /*
    Template Name: Parts Request Form
    */
    ?>
    <form action=”FormToEmail.php” method=”post” enctype=”multipart/form-data” name=”KiotiPartRequest” id=”KiotiPartRequest” onsubmit=”MM_validateForm(‘Model’,”,’R’,’Name’,”,’R’,’Address1′,”,’R’,’City.State’,”,’R’,’Zip’,”,’R’,’Phone’,”,’R’,’Email’,”,’RisEmail’,’PartDescription’,”,’R’);return document.MM_returnValue”>
    <div align=”left”>
    <table width=”90%” border=”0″ align=”center” cellpadding=”0″ cellspacing=”0″>
    <tr>
    <td width=”16%” height=”30″ valign=”top” class=”style9″>Model # </td>

    Hello,

    This will display before the Genesis Framework begins. So, it wont land within the content of the page. Have you tried pasting the html within the page content?
