Multi-Agent Parent Page

What is the correct set-up for creating a Multi-Agent page? I read about how to create the child pages that use the Multi-Agent page as a template, but I don't know if I am creating the Parent...

I tried uploading JPG's probably no less than 20 times. I went through everything you suggested and still had problems. Once I saved the files as PNG's, it worked as intended.
