That's probably a PHP syntax error. Look in your host error log.

You're probably too frustrated to want to hear this, but start over. Try adding just one field and then add the second on success. Edit just one file at a time and when the whole screen goes white find out why and fix that error.

Learning anything new under a deadline can be stressful, but you can always ask one of the StudioPress Approved Designers to give you a hand. The money that it costs will certainly outweigh the stress you have now. Many designers would be glad to show you what they did as well.
