Single post SEO title <title> to Home Doctitle

So i got a verry strange SEO question.

I want to change my default <title> on the single post pages to be just like my home doctitle i have set on wp-admin/admin.php?page=seo-settings and not the title of the post.

Only when i use the custom post title and description it must be used instead of the default.

Can this be done in functions.php and how?
