more photo issues

I'm still having a few small problems with photos. The photos show up fine on my pages, but with an extra box and a little piece of code by each one.

Also, when I create a post, the thumbnail shows up on all pages in full size, not the small thumbnail size. Is there something I need to do to set up the thumbnails? I've deleted "categories" from the header, if that makes a difference. (What I'm envisioning is no "categories" at the top, but posts that can only be accessed through thumbnails. Is the theme designed to work that way?)
