Default Lifestyle 4.0 - Can't save settings + Thumbnails missing.

I just upgraded to Lifestyle 4.0 in the hopes of solving my thumbnails issue, but now I'm having the strangest problem in the "Theme Options" section. When I set which categories I want displayed, the number of posts and thumbnail size, then click "Save Settings," the page refreshes and everything is blank (see attached image) with the same category selected for every box. This happens for every option on the Lifestyle 4.0 Theme Options Page.

I set the Tools folder cache to CHMOD 777. Is there anything else I need to do to be able to save the theme options?

Also, thumbnails still aren't working - even though I tried the solutions suggested in the thread below (not listing entire blog url just from wp-content on, checking permissions etc.)
