Unruly Bullets

I wasn't sure where to post this because I'm pretty sure it's not theme related but I keep getting bullets showing up in my sign up form on one of my landing pages and can't figure out why. I don't want the bullets next to the fields. Can anyone take a look at my code -- maybe I've been looking at it too long and I'm missing something?? Here's the link: http://www.originalginger.com/new-landing-page-alt/. Oh, and the bullets only show up in Chrome and ie8 -- not in Mozilla.
