Default Problem with Plugin

I have deactivated all plugins except PhotoSmash (http://smashly.net/photosmash-galleries/#comment-29) a cool little plugin that allows subscribers to upload their photos I have even tried it with timthumbs removed. Instead of getting the ajax upload screen that is launched via js, it goes to a 404 page.

The author of the plugin says "Could you try that using a different template. It sounds like the Javascript is not being set up properly. My guess is that the template is missing either wp_head() or wp_footer()"

When you click on the "Add Photo" link the ajax upload form should launch.

It works fine in a subdomain http://www.neothinkmembers.com/photos/ with the WP default theme.

Any thoughts, ideas and especially solutions will be greatly appreciated.
