Seeking a ZipCode to Store Location WordPress Plugin

I'm searching for a Store Locator/Dealer Locator by ZipCode WordPress plugin.

I have an existing MySQL database of stores from an old static site, upon which i'm developing a new WordPress site. I want to easily be able to configure a zipcode store locator plugin so that I can easily access this data for the new WordPress site.

But first, I need a to find a quality WordPress plugin that can search by USA ZipCode the various Store Locations. Specifically, to have the output data to be in text format. Not really concerned about a Google Map, but that would be a plus.

Inputs: ZipCode
Output: Store location within that ZipCode area.

Concept Process is like this:

Page 1: Enter Zip Code --> Click submit

Page 2: See Results in text table of store location

Anyone know of a quality, working & functioning Zip Code/Store Location plugin that is up to date?
