Header Coding

Hello All,

I've been trying to customize the header section of my site, but somehow in the process I've managed to duplicate the header somehow. You can see what I mean at www.liveandlovelifeva.com. Can someone point me in the right direction in how I might fix this?

I'm also trying to remove the 468x90 banner in the header - right now it's just a black box, but I'd like it to be removed all together.
