Theme Settings Won't Save

Theme Settings Won't Save

My theme has been doing weird things all day. Once I temporarily switched to a new theme and then back to Agent, I finally got the Wordpress Post Templates to show, but now, the theme settings have defaulted to the original values and will not save. See the attached screen grab. I had filled out most of the categories and then when I hit save, it says that it saves, but the fields are blank again. The blog title is also now appearing over my header at the top.
