Help with Threaded Comments

I upgraded WordPress to 2.7 and followed Brian's tutorial for upgrading the code for threaded comments.
It doesn't seem to be working right.

Problems:
1. It says "Comments" twice
2. There is no "reply" inside a comment for adding a threaded reply.

I just went over the tutorial again 3 times and can't figure out why it's not working correctly. I even added a new test comment to check it out.
Any ideas?
