404 Error when leaving comments

I was just testing my comments and when I or someone else tries to leave a comment we get a 'We're sorry that page is not found 404 Error" after submitting the comment. Any ideas on what is causing that?

I've unselected pagination in the Discussion panel with no success.
I've checked the Custom Permalinks for spaces with no success.
