Questionnaire plugin - any experience?

I think this is probably a non starter, but I'm looking for a plugin - or any other solution - which would enable me to add questionnaires to a site I want to build (that's not the difficult part - I use Contact Form 7 at present, for example) but then take tick box responses to questions and automatically provide feedback to the visitor. E.g.:

'You ticked mostly As, so you should you think about doing X' or

'Your score in section Y was less than Z, which means you should ...'

I'd be grateful if anyone could suggest ways of doing this.
