How to make MailChimp work

Hi all
I love Church Theme. I have just installed the MailChimp version 1.1.10 plugin. It all works and looks fine except when I go to hit the Submit button, nothing happens. It goes dead. The unsubscribe link works but not the Submit.

If I go to another page the Submit button looks active but if I click it it will do the same thing as on the home page.

My site is www.mindscriptives.com and I would appreciate sorting this out so I can get on with my site. I hate to have to switch email marketing providers.
