Default Permalink Structure Brings Down Database

I tried to change a fresh install of Church to have the permalink structure Brian suggested and go a Server Error everytime (had to reinstall). I have Dean's Permalink Plug-in installed.

Tried to change to /%postname%.html but it brings the server down and even I try to change back to default I still get the server error....having to reinstall each time.....I've read through the forums and it looks like the Permalink plug-in should help.....but it's not:
