What Banner Ad Code Should I Use?

Hi,

New to coding and just starting working on my lifestyle blog 2 days ago. I'd like to add an image to the rt side of my header(where the banner goes). I created a 468x60 Banner Ad in Photoshop and uploaded it to my hosting acct images directory and titled it: UC.png. Can someone help me and post the code that I should copy and past into the Rev Lifestyle Options General Settings text box. I know where the code goes just don't know what the code should be. When I uploaded the video youtube generated the code for me and I can't figure out how to do this image banner code.
