Looking to hire Wordpress developer

I have an HTML form that I want to embed within a Genesis themed Wordpress site. I know you have to create a custom template but that's about where my knowledge ends.

So I'm looking to hire someone to help me with this. The site is a very simple e-commerce site with a single form that currently coded in HTML. When submitted, the form data is passed to a hosted payment gateway.

Code must be clean and well commented. The need is immediate.
