Default Help coding Plug-in in Primary Sidebar

I am trying to put a survey onto my sites. More specifically, I would like to have a different survey on each page and have lots of surveys in several of my sites. I am experimenting first with this site, www.chiropracticneurologist.org
I am a novice and was expecting that I could just plug this plug-in like other widgets once the survey was configured and ShaZam!, but alas....It does not give me a "widget" to drop into the primary sidebar. I guess that means I have to know were to insert code to get it to work. I'm new to all this but willing to wade-in.

I have already installed a plugin and designed a survey. The instructions after configuring the WPSS plugin state:

Update settings and insert: [wp-simple-survey]

into your content, where you want the quiz to appear.


On the plug-in website, Alicia St Rose suggested some script be inserted into the "functions.php".

WP Simple Survey which has the following description:
A jQuery-based plugin that displays basic weighted survey, and then routes user to location based on score. Survey displays one question at a time, and uses jQuery to reload the subsequent question without reloading the page. Scores, Names, and Results can be recorded, emailed, and displayed in the Wordpress backend.

You may have a better idea in terms of achieving my goal. My goal is to capture contact information and data empowering me to email suggestions which the potential customer can then use as a guide to purchase products which may help them. A the very least, they can get some information which helps them understand that they have a need and that there is hope.
