Customer Form - What is this one?

Hi There

I've been using Fast Secure Contact Form and its great but - it doesnt look that great on a customer page eg the boxes are small.

I found this wp site and the contact form is called 'quote-form' according to the source but I can't find it

Do you have any idea what this plugin is or what is similar to this actions eg adding the google map and having a nice looking form set up?
