Default Hacked by tiger-m@te

I was on of the sites (yet on a dedicated server) hacked by tiger-m@te this weekend. What kinds of things should I do to help eliminate this from happening again. The hacker removed/replaced the index.php or index.html plus added his index.php to most all public folders.

Need some recommendations. The hacker got to us through our provided and not one of our websites.

We've gotten most everyone back up, but need to have a plan to defined us from even our provider.

HELP!!!!
