Easy Form Widget That Works Well With Theme?

I know that there's a ton of widgets out there for creating your own forms, but so far I have yet to come across one that's simple and easy to install and use. I want one where I can easily input what fields I want (basic stuff - contact information), and have it sent to an email address of my choosing, and be able to write what it says to users when they submit it to me (the whole 'thanks for submitting, we have received your message' sort of thing). Is there any such widget - particularly one for AgentPress - that exists?
