Moving opt-in box for subscriptions to header

Hi,

I'd like to add a client's opt-in box for their newsletter to the header. I'm thinking I just take the optin code and paste it into the header.php file, but not sure where & do I need to add open/close div tags?
Here's what I want it to look like:[attachment=0:1nfo89uh]test.jpg[/attachment:1nfo89uh]
