Is it a form or a database?

How can I create a page that functions like the link I am providing here into WordPress:
http://www.trainwithmeonline.com/nutrition_myplan.asp

My Nutrition Guide,
Age:
Sex:
Physical Activity:

You input these parameters and get back results.

Is it a MYSQL database you convert with the 'X Data Toolkit' plugin,
or is there a simpler solution.
